General Practice and Cardiology:

The role of General practice is being increasing in the management of the cardiovascular diseases. A general practitioner should be able to manage patients with mild to moderate heart failure and patients with atrial fibrillation in Primary care. Minor surgery in general practice is being practiced since long time and it is cost effective and patient friendly. Minor surgery processes such as removing skin tags can be performed by the General Practitioner. Minor surgery falls into two distinct procedures excision followed by suturing and injections. A general practitioner should be properly equipped and follow standard guidelines during minor surgery to avoid further infections and complications. General Surgery is a preliminary surgery. In later stages of initial diagnosis by general medicine doctor, the patient may be suggested for surgery that is, operating and dissecting human body in process of treatment.

General Practice and Neurology:

Neurological problems in general practices are common and makes up around 7% of consultations. Neurological illnesses are frequently disabling and only rarely do they go away. General practitioners will diagnose and manage the most common conditions, like headache and migraine. They refer for diagnosis less common ones, like epilepsy and Parkinson's disease, and take most of them back afterwards for long-term management.

A General practitioner should describe the ethical principles involved when treating patients who have sexual health concerns, e.g. contraception and abortion, Understand the different cultural expectations regarding sexual behavior and orientation, Describe the importance of confidentiality, informed choice and valid consent, Be aware of the legal aspects, Ensure sensitivity to particular cultural beliefs and patient choice, e.g. the need for a female practitioner.

Ear, Nose and throat Diseases form a major part of the General Practice consultations. In adults external ear problems especially wax are the common reasons. In older children Otitis media is the most common presentation. A general practitioner should manage primary contact with patients who have ENT problems, identify symptoms that are within the normal range and in the severe cases refer them to an ENT specialist.

General Practice and Infectious disease:

General Practitioners are being more than ever in managing the complicated infectious diseases. There are too few infectious disease specialists to meet the demand, particularly in rural areas. Complicating issue like increasing antibiotic resistance, lack of new antimicrobials are the most serious health threats. General Practitioners may greatly improve the management of the patients with infectious diseases.

General Practice and Mental health:

Mental health issues are recognized as a major public health problem today and the management of mental health problems plays an increasing burden on health services. General practitioners are successful in managing uncertainty in clinical practice. It is not always necessary to make a diagnosis in order to make a management decision, or determine a course of treatment. It is important that general practice and primary care develop their own philosophies and management with regard to psychological disorder, which takes uncertainty and our present imperfect understanding of symptoms and disorders into account.

Respiratory diseases are most common long-term conditions affecting people. The identification, assessment, diagnosis and treatment of most respiratory diseases is a primary care issue. The impact of respiratory medicine on patients, families, health services and society is significant. Emergency care is a role of general practitioners and they should be properly educated, equipped skilled and organize their practices to provide prompt and effective management whenever the need needed. Writing an appropriate referral, arranging transport facilities, informing the hospital about the referral are also important steps in the process as these measures could prevent crucial delays. Emergency care or nursing should be given to patients in life threatening cases to save the life of individual. It is a medical treatment provided to effected or injured person based on their medical conditions.

Market Analysis:

According to the study conducted by Deloitte Funding for general practice is due to fall by 17% in real terms by 2017/18, while patient consultations are set to rise by 69m. the study shows that if current trends continue, funding for general practice across the UK will fall to a record low of just 7.29% of the NHS budget by 2017/18 – down from 8.39% in 2012/13. The number of consultations in general practice has increased as the population has increased in size, grown older, and more and more people have multiple-conditions. However, the share of funding spent on general practice has been falling since 2005/6. In the report, Deloitte projects that funding for general practice will fall from £9.29bn of the current total NHS budget of £110.9bn to only £7.7bn, in today’s money, by 2017/18, a decline of 17.1%.

In a recent poll of members of the public, conducted by ComRes, in March, 60% of the public said they thought that some funding should be moved from other parts of the health service into general practice.

Deloitte estimates that even to stand still, general practice would need £11.47bn – or 9.81% – out of a projected NHS budget of £116.86bn by 2017/18.

According to an article published in Mirror the share of funding for general practice has dipped from 10.6% in 2005/06 to 8.2% in 2013/14.
